What are All-on-4 dental implants?

All-on-4 is a full-arch treatment where four implants support a fixed bridge of 10 to 14 teeth. The two back implants are tilted to use existing bone, which often avoids bone grafting. At Gangnam One Day Dental Clinic, a fixed temporary arch can usually be attached on surgery day, and the final arch follows after typically 3 to 6 months.

How All-on-4 works

Two implants are placed straight in the front of the jaw and two are tilted at the back. Tilting lets longer implants anchor in denser front bone and avoids the sinus in the upper jaw and the nerve in the lower jaw. A rigid bridge then links all four implants so they share the bite load.

Remaining failing teeth are usually removed during the same surgery. Our published cases include 8 implants placed in 35 to 40 minutes and 15 implants in 1 hour, though surgery time always depends on difficulty.

Four implants or more?

Four implants is a minimum, not a rule. Where bone is soft, the arch is wide, or bite forces are high, the dentist may recommend 5 or 6 implants per arch for extra support. This is decided from your 3D CT and bite, not from a package.

If you prefer something removable or need a lower-cost option, an implant-supported denture on 2 to 4 implants is an alternative. If many of your own teeth can still be saved, a full-mouth reconstruction that mixes crowns and implants may suit you better.

  • 4 implants: enough bone in the front jaw, average bite force
  • 5–6 implants: soft bone, wide arch, strong bite or grinding
  • Overdenture on 2–4 implants: removable option, easier cleaning

Protecting the bite on a full arch

A full arch on implants has no natural tooth ligament to cushion bite forces, so uneven contacts can chip the teeth or overload one implant. Chief Director Dr. Kim Jin-hwan, author of a handbook chapter on controlling the occlusion of implant-supported restorations, uses T-Scan Novus to measure and balance the bite.

The final arch is made in our Oneday Lab, usually from zirconia or a reinforced framework with ceramic teeth. See how timing works on our same-day implants page.

How it works at Oneday Dental

  1. Before travel

    CT or panoramic review

Send a CT or panoramic X-ray on WhatsApp. The dentist outlines whether All-on-4 is feasible and how many trips you need.

  2. Day 1

    3D CT, scans and design

3D CT, intraoral scans and photos are used to plan implant angles and design the temporary arch.

  3. Day 2

    Extractions and implants

Failing teeth are removed and the implants placed, with IV sleep sedation if you choose.

  4. Day 2–3

    Fixed temporary arch

If the implants are stable, a fixed temporary arch made in Oneday Lab is attached.

  5. 3–6 months

    Final arch

The final arch is fitted and the bite balanced with T-Scan Novus.

Things to know before you decide

  • Very little bone even in the front jaw may require grafting or a different approach.
  • The temporary arch needs a soft diet for about 2–3 months; chewing hard food risks implant failure.
  • Heavy grinding can fracture the prosthesis; a night guard is often recommended.
  • Cleaning under a fixed arch needs a water flosser or special brushes, and check-ups every 6 months.
  • Smoking and uncontrolled diabetes raise the risk of implant failure.

FAQ

All-on-4 Implants: common questions

Is All-on-4 suitable for me if I already wear dentures?
Often yes. Many denture wearers choose All-on-4 to get fixed teeth that do not move or cover the palate. Long-term denture wear can shrink the bone, so a 3D CT is needed to confirm there is enough bone at the four implant sites.
How many trips to Korea does All-on-4 need?
Usually two. The first trip, typically about 5 to 7 days, covers examination, surgery and the fixed temporary arch. The second trip, after 3 to 6 months, is for the final arch and bite adjustment. The exact schedule is confirmed after your CT review.
How much does All-on-4 cost in Korea?
The cost depends on whether one or both arches are treated, the implant system, the final prosthesis material, extractions and any grafting. Send your CT or panoramic X-ray on WhatsApp and the dentist will give an estimate range, confirmed after examination.
Can I be asleep during All-on-4 surgery?
IV sleep sedation is available for implant surgery. You are drowsy and relaxed and usually remember little of the procedure, while local anaesthesia numbs the area. You need an adult companion afterwards and should not drive that day.
How long do All-on-4 teeth last?
The implants themselves can last many years with good cleaning and check-ups. The prosthetic teeth wear over time and may need repair or replacement, often after 10 to 15 years depending on material and bite forces.
Which is better, All-on-4 or All-on-6?
Neither is better for everyone. Four implants can avoid bone grafting; six give more support where bone is soft or bite forces are high. The dentist recommends a number based on your 3D CT and bite analysis.
