What should I do if my dental implant feels loose?

First, find out what is loose: the crown, the abutment screw, or the implant itself. A loose crown or screw is usually repairable in one visit. A loose implant in the bone typically means it has failed and needs removal and replacement. Gangnam One Day Dental Clinic diagnoses this with X-ray or 3D CT and T-Scan bite analysis. Avoid chewing on it meanwhile.

Which part of the implant has a problem?

An implant has three parts: the fixture in the bone, the abutment screwed into it, and the crown on top. The fix depends entirely on which part is affected, so diagnosis comes before any repair.

A crown that rocks but an implant that stays still usually means a loose screw or failed cement. A chipped crown can be repaired or remade in our Oneday Lab, often within the same visit. If the implant itself moves, it has usually lost its bond with the bone.

  • Loose or rotating crown: abutment screw loosening or cement failure
  • Chipped or cracked crown: often caused by bite overload
  • Red, bleeding or swollen gum around the implant: peri-implant mucositis or peri-implantitis
  • Food trapping next to the implant: contact gap with the neighbouring tooth
  • Implant moves as a whole: loss of osseointegration

Why bite overload is often the hidden cause

Natural teeth sit in a ligament that lets them move slightly; implants do not. If an implant crown touches too early or too hard, it takes extra force with every bite. Over time this loosens screws, chips porcelain and can contribute to bone loss.

Chief Director Dr. Kim Jin-hwan wrote the chapter "Controlling the Occlusion of Implant-Supported Restorations" in the IGI Global Handbook of Research on T-Scan Technology Applications in Dental Medicine. We use T-Scan Novus to measure exactly when and how hard each tooth contacts, then adjust the bite so the implant is not overloaded. Grinding may also need a night guard.

Implants placed at another clinic or abroad

You do not need to have had your implant placed here to have it checked. Repairs are easier when we know the implant brand and connection type, so bring any implant card, records or X-rays you have. We use OSSTEM and MegaGen; parts for some other systems may need to be ordered, or the crown may need a new design.

Gum inflammation around implants is treated by our periodontics team, using deep cleaning, laser treatment or surgery depending on bone loss. If an implant must be replaced, see our dental implants page for how new placement works.

    Diagnosis

Clinical exam, X-ray or 3D CT to check the bone, and T-Scan Novus to check bite forces.

  3. Visit 1

    Find the cause

The dentist explains which part has failed and why, and what repair options exist.

  4. Visit 1–2

    Repair

Screw replacement, crown repair or remake in Oneday Lab, gum treatment, or replacement planning.

  5. Ongoing

    Maintenance

Bite check and cleaning every 6 months help prevent the problem from returning.

Things to know before you decide

  • Some implant brands used abroad have parts that are hard to source, which can mean a new abutment and crown design.
  • Advanced peri-implantitis with major bone loss may mean the implant cannot be saved.
  • Removing a failed implant usually requires a healing period, and sometimes grafting, before a new one is placed.
  • Repairs without correcting the bite or grinding often fail again.
  • Smoking and poor cleaning around implants increase the risk of gum and bone problems.

FAQ

Implant Repair: common questions

Can you fix an implant that was placed in another country?
Usually yes. We examine it with X-ray or 3D CT and check the bite. Knowing the implant brand helps us find compatible parts. If parts are unavailable, a custom abutment or a redesigned crown may be needed, which the dentist explains before starting.
My implant crown is loose. Is the implant failing?
Not necessarily. Most loose crowns are caused by a loosened abutment screw or failed cement, and the implant in the bone is fine. These problems can often be fixed in one visit. An X-ray confirms whether the bone around the implant is healthy.
What is peri-implantitis?
Peri-implantitis is an infection of the gum and bone around an implant, similar to gum disease around a tooth. Signs include bleeding, swelling, pus or a deepening pocket, and bone loss on X-ray. Early cases respond to cleaning; advanced cases may need surgery.
Why does my implant crown keep chipping?
Repeated chipping usually points to bite overload, often from grinding or a crown that contacts too early. We measure this with T-Scan Novus and adjust the bite, may choose a stronger material such as zirconia, and often recommend a night guard.
How much does implant repair cost?
It depends on the problem: retightening a screw costs far less than remaking a crown or replacing an implant. Send photos and an X-ray on WhatsApp for a provisional estimate range, which the dentist confirms after examination.
